In presenting yourself for Phytotherapy (Western herbal medicine) treatment as practised by Sebastian Liew Centre Pte Ltd (SLC) or consumption of any of our health supplements, you acknowledged and agreed that it is intended to promote health, support healthy functions, and general wellness. 

Phytotherapy does not diagnose disease and does not prescribe drugs or surgery. You understand that Phytotherapy is not a substitute for medical treatment or medications and that it is recommended that you concurrently work with your primary health provider for any disease condition. 

If you are using pharmaceutical medications of any kind, you are required to alert the medical herbalist or naturopath and discuss possible herb-drug interactions. If you are suffering from any diagnosed psychiatric conditions, you must inform the herbalist or naturopath. 

All herbal remedies or supplements recommended must not be taken together with any pharmaceutical medications. If in doubt, please seek professional advice. You voluntarily consent to receive Phytotherapy or consumption of our health supplements, and you have informed your primary health care provider. 

You have also informed the herbalist or naturopath of all your known physical conditions, medical conditions, and medications. And you will keep your primary health care provider, or the medical doctor, updated on your health condition.
